De zorggebruikcategorie wordt bepaald o.b.v. aantal gewogen contacten in de afgelopen 12 maanden. Weging van contactsoorten volgens Nivel:
- 0,5 = Telefonisch consult, E-consult en kort consult
- 1 = Regulier consult
- 1,5 = Reguliere visite
- 2 = Langdurig/dubbel consult
- 2,5 = Langdurige/dubbele visite

Aandoeningen uit de frailty index
De Frailty Index is opgebouwd op basis van zogenaamde aandoeningen, zie definities hieronder.
Voor de Frailty Index worden 50 aandoeningen genoemd. Een aandoening scoort positief wanneer tenminste één van de items van de bijbehorende Episode/Medicatie aanwezig is in een bepaalde periode vóór de peildatum. Deze periode is gelijk aan de periode die in de kolom meetperiode is vermeld.
Alle positieve aandoeningen opgeteld en gedeeld door 50 geeft de Frailty Index. Deze kan dus variëren van 0 tot 1. Hoe meer deficits aanwezig zijn bij een patiënt hoe hoger de Frailty Index is.
